EARLY ENROLLMENT PROCEDURE FOR INCOMING UNDERGRADUATE
STEP 1: SUBMISSION OF ACCOMPLISHED AND SIGNED TRASTS FORMS

• Notarized Provisional Enrollment Form
• Advising & Crediting Form from the DLSMHSI College Secretary (for Transferee Applicants)
• Completed and signed Acknowledgement Letter (for Filipino Applicants)
• Completed and signed Acknowledgement Letter (for Foreign Applicants)
• Completed Applicants Personal History Statement
• Completed and signed Special Needs Assessment Form
• Completed and signed Declaration of Citizenship

STEP 2: CHOOSING MEDICAL EXAMINATION SCHEDULE & UNIFORM MEASUREMENT
All incoming freshmen are required to undergo a Medical Examination at De La Salle University Medical Center (DLSUMC) as a part of the Admission Requirement. On the same day, all incoming applicants may have their Uniform Measurement.

STEP 3: EARLY ENROLLMENT
Once you've received a confirmation email, please log in to your applicant's portal for the Online Enrollment Procedures.
After receiving your Registration Form, please like and follow our official Facebook page (DLSHSI Admission and Scholarships) for further announcements like the Start of Class, College and Institutional Orientation, Academic Calendar, updates, etc.
Note:
Once we release the final tuition fee, you can pay the remaining balance for you to be considered full payment (no installment fee) until the last day of our regular enrollment date.

STEP 4: SUBMISSION OF ORIGINAL REQUIREMENTS
Please prepare the following printed original requirements on or before your Preliminary Examination (exact date shall be announced) with the following information, so that you can take your Preliminary Examination for the 1st semester.
• Requirements (for applicants from DLSMHSI Senior High):
º Photocopy of Birth Certificate
º Photocopy of High School Diploma or Certificate of Graduation
• Requirements (for applicants from other schools):
º Photocopy of Birth Certificate
º Original Form 138 (Grade 12 High School Report Card)
º Photocopy of High School Diploma/Certificate of Graduation
• Requirements (for transferee applicants):
º Photocopy of Birth Certificate
º Original Transfer Credential/Honorable Dismissal from the previous school
º Completed and signed Advising & Crediting Form from the DLSMHSI College Secretary
Additional Requirements for Foreign Applicants:
º Photocopy of passport.
º Photocopy of Alien Certificate of Registration (ACR) for permanent residents, native born, children of diplomats, or holder of any valid visa and Certificate of Residence for Temporary Students (CRTS).
º Special Study Permit (SSP) for foreigners studying in the Philippines for less than a year or less than 18 years old (for foreign applicants).
º Certificate of Completion for Improving Tagalog Communication Skills (Special Filipino Class) from Institute’s Center for Internationalization, Academic Affiliations and Engagements (CIAAE) (for Nursing applicants)
