EARLY ENROLLMENT PROCEDURE FOR INCOMING UNDERGRADUATE
STEP 1: SUBMISSION OF ACCOMPLISHED AND SIGNED TRASTS FORMS
Email us the following requirements at TRASTSrequirements@dlsmhsi.edu.ph (click each form to download).
• Notarized Provisional Enrollment Form
• Advising & Crediting Form from the DLSMHSI College Secretary (for Transferee Applicants)
• Completed and signed Acknowledgement Letter (for Filipino Applicants)
• Completed and signed Acknowledgement Letter (for Foreign Applicants)
• Completed Applicants Personal History Statement
• Completed and signed Special Needs Assessment Form
• Completed and signed Declaration of Citizenship
Specifying the following:

STEP 2: CHOOSING MEDICAL EXAMINATION SCHEDULE & UNIFORM MEASUREMENT
As preparation for limited face-to-face classes for SY 2025-2026, all incoming freshmen must undergo a Medical Examination at De La Salle University Medical Center (DLSUMC) as a part of the Admission Requirements. On the same day, all incoming applicants may have their Uniform Measurement.

STEP 3: EARLY ENROLLMENT
Once you've received a confirmation email, please log in to your applicant's portal for the Online Enrollment Procedures.
After receiving your Registration Form, please like and follow our official Facebook page (DLSHSI Admission and Scholarships) for further announcements like the Start of Class, College and Institutional Orientation, Academic Calendar, updates, etc.
Note:
Once we release the final tuition fee, you can pay the remaining balance for you to be considered full payment (no installment fee) until the last day of our regular enrollment date.
