STEP 1: RESERVATION
This is to formally inform you that your application has been approved. You can pay your reservation fee through Dragonpay (see the link for the bank details and video for the payment procedures) to confirm your slot. Please note that the slots are limited. The deadline for reservation of your slot is one (1) month after receiving this email or until slots are available.
If you haven’t received your Official Receipt from our cashier, you can email our cashier directly to ask for updates at proofofpayment@dlsmhsi.edu.ph or call them at Cavite line: (6346) 481-8000 | Manila line: (632) 8988-3100 local 1433
STEP 2: SUBMISSION OF ACCOMPLISHED AND SIGNED TRASTS FORMS
Email us the following requirements at kmgabiana@dlshsi.edu.ph (click each form to download).
• Proof of payment (Official Receipt from our cashier)
• Notarized Provisional Enrollment Form
• Completed and signed Acknowledgement Letter (for Filipino Applicants)
• Completed and signed Acknowledgement Letter (for Foreign Applicants)
• Completed and signed Applicants Personal History Statement
• Completed and signed Special Needs Assessment Form
• Completed and notarized Provisional Enrollment Form
• Completed and signed Information Sheet

STEP 3: MEDICAL EXAMINATION & UNIFORM MEASUREMENT
All DLSMHSI studens are required to undergo Medical Examination at De La Salle University Medical Center (DLSUMC) as a part of the Admission Requirement.

SUBMISSION OF ORIGINAL REQUIREMENTS
Please submit the following printed requirements on the specified date personally or via snail mail/courier with the following information so we can update your clearance in your portal and for you to enroll for the next semester:
• Requirements:
º Original Birth Certificate
º Original Form 138 (Grade 10 High School Report Card)
º Photocopy of Certificate of Moving-up
º Certificate of Moral Character issued by the Student Affairs and its equivalent
