CONFIRMATION PROCEDURES

STEP 1: Fill out the Online Application Form (click "Applying for College or Post Graduate" then “Cross Enrollee”).

Note: for applicants who already have a DLSMHSI student ID number (e.g. DLSMHSI graduates), please follow the link below:

STEP 2: Input your temporary ID number/student number (for applicants from DLSMHSI SH), full name, and email address using the Google Form

STEP 3: Please wait for the email from admissionscm@dlsmhsi.edu.ph to give you instructions to log in to your applicant’s portal and choose your medical examination.

STEP 4: Submit the following original requirements to DLSMHSI, Center for Lasallian Admissions, Scholarships and Testing Section (CLAST) located at Lourdes Building, Ground floor, on the date of your Medical Examination (look for Ms. Sarah).

Requirements

• Photocopy of Birth Certificate

• 1 pc. 2×2 picture (white background)

Additional Requirements for Foreign Medicine Applicants

• Photocopy of passport/ Birth Certificate / NBI / Bureau of Quarantine / CEA

• Photocopy of Alien Certificate of Registration (ACR) for permanent residents, native-born, children of diplomats, or holders of any valid visa and Certificate of Residence for Temporary Students (CRTS)

• Special Study Permit (SSP) for foreigners studying in the Philippines for less than a year or less than 18 years old (for foreign applicants).

• Notarized Affidavit of Support to cover for student’s accommodation and bank certification, Certificate of Completion for Improving Tagalog Communication Skills (Special Filipino Class) from the Institute’s Center for Internationalization, Academic Affiliations and Engagements (CIAAE)

STEP 5: Enroll (check your applicant's portal for the complete Enrollment Procedures & Enrollment date).

Note: All foreign students are required to pay the $7,000.00 foreign fee upon enrollment.
