STEP 1: RESERVATION

This is to formally inform you that your application has been approved. You can pay your reservation fee through Dragonpay (see the link for the bank details and video for the payment procedures) to confirm your slot. Please note that the slots are limited. The deadline for reservation of your slot is one (1) month after receiving this email or until slots are available.

STEP 3: MEDICAL EXAMINATION & UNIFORM MEASUREMENT

As preparation for limited face-to-face classes for SY 2026-2027, all incoming freshmen are required to undergo Medical Examination at De La Salle University Medical Center (DLSUMC) as a part of the Admission Requirement. On the same day,  all incoming applicants may have their Uniform Measurement.

STEP 5: SUBMISSION OF ORIGINAL REQUIREMENTS

Submit the following original requirements through snail mail/courier or personal visit on the schedule to be given to you by The Registrar - Admissions, Scholarships, and Testing Section (TRASTS).

º Original NMAT Result

º Photocopy of Birth Certificate

º Certificate of Graduation or Photocopy of Diploma

º Original Notarized Sworn Statement

º 1 pc. 2x2 picture (white background)

• Requirements (applicants from other school):

º Transfer Credential / Honorable Dismissal from previous school

º Original NMAT Result

º Photocopy of Birth Certificate

º Certificate of Graduation or Photocopy of Diploma

º Original Certificate of General Weighted Average (GWA) / Certificate of General Pointed Average (GPA)

º Original Notarized Sworn Statement

º Certificate of Moral Character issued by the Student Affairs and its equivalent

º 1 pc. 2x2 picture (white background)
