RESERVATION PROCEDURE FOR INCOMING UNDERGRADUATE
STEP 1: RESERVATION
This is to formally inform you that your application has been approved. You can pay your reservation fee through Dragonpay (see the link for the bank details and video for the payment procedures) to confirm your slot. Please note that the slots are limited. The deadline for reservation of your slot is one (1) month after receiving this email or until slots are available.

STEP 2: SUBMISSION OF ACCOMPLISHED AND SIGNED TRASTS FORMS
Email us the following requirements at TRASTSrequirements@dlsmhsi.edu.ph (click each form to download).
• Proof of payment (Official Receipt from our cashier)
• Completed and signed Acknowledgement Letter (for Filipino Applicants)
• Completed and signed Acknowledgement Letter (for Foreign Applicants)
• Completed Applicants Personal History Statement
• Completed and signed Special Needs Assessment Form
• Completed and signed Declaration of Citizenship

STEP 3: MEDICAL EXAMINATION & UNIFORM MEASUREMENT
As preparation for face-to-face classes, all incoming freshmen are required to undergo a Medical Examination at De La Salle University Medical Center (DLSUMC) as a part of the Admission Requirement. On the same day, all incoming applicants may have their Uniform Measurement.

STEP 4: SCHOLARSHIP APPLICATION
The Registrar – Admission, Scholarship, and Testing Section (TRASTS) of DLSMHSI provides every Filipino student a unique opportunity to experience free Lasallian health sciences education. STEP 5: SUBMISSION OF ORIGINAL REQUIREMENTS
Please submit the following printed requirements on or before your enrollment date via snail mail/courier with the following information, so we can update your portal and receive your Enrollment Procedures on your portal.
• Requirements (for applicants from DLSMHSI Senior High):
º Photocopy of Birth Certificate
º Photocopy of High School Diploma or Certificate of Graduation
• Requirements (for applicants from other schools):
º Photocopy of Birth Certificate
º Original Form 138 (Grade 12 High School Report Card)
º Photocopy of High School Diploma/Certificate of Graduation
• Requirements (for transferee applicants):
º Photocopy of Birth Certificate
º Original Transfer Credential/Honorable Dismissal from the previous school
º Completed and signed Advising & Crediting Form from the DLSMHSI College Secretary
Additional Requirements for Foreign Applicants:
º Photocopy of passport.
º Photocopy of Alien Certificate of Registration (ACR) for permanent residents, native born, children of diplomats, or holder of any valid visa and Certificate of Residence for Temporary Students (CRTS).
º Special Study Permit (SSP) for foreigners studying in the Philippines for less than a year or less than 18 years old (for foreign applicants).
º Certificate of Completion for Improving Tagalog Communication Skills (Special Filipino Class) from Institute’s Center for Internationalization, Academic Affiliations and Engagements (CIAAE) (for Nursing applicants)

STEP 6: ENROLLMENT
Once you submitted the original documents, please log in to your Applicant's portal for Enrollment procedures. If you just emailed the Notarized Provisional Enrollment Form (for Temporary Enrollment), please wait for the confirmation email first before logging in to your applicant's portal.
