STEP 1: RESERVATION
This is to formally inform you that your application has been approved. You can pay your reservation fee through Dragonpay (see the link for the bank details and video for the payment procedures) to confirm your slot. Please note that the slots are limited. The deadline for reservation of your slot is one (1) month after receiving this email or until slots are available.

STEP 2.1: SUBMISSION OF ACCOMPLISHED AND SIGNED TRASTS FORMS
Email us the following requirements at postgradadmission@dlshsi.edu.ph (click each form to download).
• Proof of payment (Official Receipt from our cashier)
• Notarized Provisional Enrollment Form
• Completed and signed Acknowledgement Letter
• Completed and signed Applicant Personal History Statement
• Completed and signed Special Needs Assessment Form
• Completed and signed Declaration of Citizenship

STEP 2.2: SCHOLARSHIP APPLICATION

Submit the following original requirements thru snail mail/courier or personal visit on the schedule to be given to you by the Lasallian Admission and Scholarship Opportunities.
• Requirements:
º Transfer Credential / Honorable Dismissal from the previous school
º Photocopy of Birth Certificate
• Additional Requirements for Foreign Post-Graduate applicants
º Photocopy of passport / Birth Certificate / NBI / Bureau of Quarantine / CEA
º Photocopy of Alien Certificate of Registration (ACR) for permanent residents, native-born, children of diplomats, or holders of any valid visa and Certificate of Residence for Temporary Students (CRTS)
º Special Study Permit (SSP) for foreigners studying in the Philippines for less than a year or less than 18 years old (for foreign applicants).
º Notarized Affidavit of Support to cover for student’s accommodation and bank certification
