If you have received a Midterm Grade Report from your instructor, please submit it via the link below.
Deadline: March 8, Friday
Absent for a class?
Download OVCA Form 113 here.
Accomplish the form and attach supporting document/s.
*If the supporting document is a letter from your parent/guardian, attach a copy of their ID with 3 specimen signature. Only the name and signature on the ID must be visible. Any other personal information should be hidden, in compliance with the Data Privacy Act.
Email your accomplished form to casdean@dlsmhsi.edu.ph. Include the following information in your email:
a. Your name and section
b. Instructors affected by your absence
c. A short narrative on why you were absent
d. A subject line
Form must be submitted within 2 school days upon return to class.
