REMINDER: State of Illinois Kindergarten Health Compliance Requirements include a physical exam performed in the last 12 months, proof of immunizations, and eye exam and a dental exam. All documents need to be submitted before October 15th, 2020. Please do not delay making an appointment if you have not already completed these exams. Physician offices book up very quickly this time of year due to these statewide requirements. Do not hesitate to contact me if you have any questions!
