Anytime that  you have new applications that are ready to be submitted, log in with a Google Account, then use the CSO Documents Uploads Form below.

1. Click FILL OUT FORM

2. Select SGO INCOME VERIFICATION DOCUMENTS as the information type you are submitting.

3. Click NEXT, then ADD FILE 

4. The next step is to select each file you wish to upload from your computer. Once each has been selected click UPLOAD.  (If you missed any you may click ADD FILE again,  and add the ones missed.)

5. When all have been uploaded, the final step is to click on SUBMIT.  After a few seconds, once your files are uploaded, you will receive a confirmation message letting you know your files were uploaded successfully.
