REQUIRED FORMS to Participate in DPMS Athletics

1. WIAA Clearance Form- Sports Physical Form

• All students planning to participate in a middle sport during the 2024-2025 school year must have a physical on file at the school, dated after April 1, 2023, prior to starting official practices.

2. Athletic Participation Form *complete during online school registration

5. Athletics Fee

• • • $21/year, regardless of the number of sports. Pay online through the parent portal.   Go to Fees->My Account (upper right hand corner- be sure to check the right student)->Optional Payments->MS Athletics.
