WELCOME TO DEPED TARLAC PROVINCE SHS ONLINE APPLICATION SUBMISSION PORTAL

All applicants shall submit their application online on this portal.

Before filling up the form, please read the following:

  1. You must have a reliable internet connection to fill up the form completely and upload your documents.

  2. Scan or take photos (must be clear enough to read) of your documents and must place in one folder using your Full Name (ie. JUAN DELA CRUZ) as filename.

  3. Compress the folder in RAR or ZIP Format.


Folder must contain the following:

a. Application Letter addressed to the Schools Division Superintendent (please indicate Track/Strand)

b. Authentication and veracity of all documents submitted

c. Personal Data Sheet (CS Form No. 212, Revised 2017)

d. Transcript of Records (Baccalaureate)

e. Master’s Degree Transcript of Records/Certification of Units Earned (at least 6 units towards a Master’s degree in relevant strand/subject for Academic Track and Core Subjects) or Certificate of Registration if currently enrolled.

f. Doctorate Degree Transcript of Records/Certification of Units Earned, if any.

g. Certification from the School of General Weighted Average (GWA), if none, the applicant must present a computation of GWA.

      1. d.1 For Education Graduates – GWA of baccalaureate degree

      2. d.2 For Non-Education Graduates – GWA of baccalaureate degree and the eighteen (18)

      3. professional units in education if there is any,

h. Copies of Service Records or Certificate of Employment with Inclusive Dates, Performance Rating and School Clearance for those with teaching experience.Applicants with teaching experience

i. Certificates of Seminars/Trainings attended after the baccalaureate degree with inclusive dates

j. Certificate of Specialized Training, if any, or National Certificate issued by TESDA;

k. Teacher Methodology Certificate for TVL applicants;

l. Certified copy of Voter’s ID and/or any proof of residency as deemed acceptable by the School Screening Committee.


Instruction:

  1. Select the strand/track you wish to apply.

  2. Login with your GMAIL/ GOOGLE account. This form can only be filled once using your Google Account. Strictly One Google Account per Applicant

  3. Fill out the form properly and accurately. Details submitted must be true and correct.
