Division and Unit/Section (Curriculum Implementation Division, Office of the Schools Division Superintendent, or School Governance Operations Division)
Program Information, including the Title, Start Date, Duration, and Venue of the Activity
Supporting Documents, such as the Training Matrix/Program and Approved Program Proposal or Authority to Conduct, both required to be uploaded in PDF format
