What should I do when submitting documents to the RMU?
Ensure that the documents are complete, properly arranged, and addressed to the appropriate office/person. Documents should be submitted through the prescribed receiving procedure for proper recording and tracking. For schools within SDO Laguna, attach duly filled out Attestation Form along with the document/s to be submitted.
How can I check the status of my submitted document?
You may inquire with the RMU using the receiving/reference number indicated on your acknowledgment receipt or other proof of submission.
How long does it take to process a requested document?
Processing time depends on the type of document, completeness of requirements, verification needed, and applicable office procedures. The prescribed processing time shall be observed in accordance with existing service standards.
What should I bring when claiming a document?
a. If you're the Liaison Officer (LO), please bring 1 Valid ID for verification purposes
b. If you're the Representative, please bring the following:
i. Signed Authorization Letter of the Document Owner
ii. Photocopy of ID of the Document Owner with 3 Specimen Signatures
iii. Photocopy of ID of the Representative with 3 Specimen Signatures
*For Legal/ Confidential Documents, it is recommended that the owner shall claim the records/documents.
CERTIFICATION, AUTHENTICATION, AND VERIFICATION (CAV)
How do I request certification, authentication, or verification of a school record?
CAV Process is the responsibility and function of DepEd Regional Offices. Private and Public Schools (Open and still operating) shall assist learner/s in providing requested Academic School Record/s (ASR) which will be required for CAV
What academic school records may be subject to certification or authentication?
Records such as Form 137/SF10, Form 138/SF9, certificates, diplomas, school forms, and other official school records may be processed when authorized and when the records are available and verifiable.
What if the school record cannot be found?
The RMU or concerned office may coordinate with the appropriate school or records custodian to verify the availability and authenticity of the requested record.
Can diploma be reconstructed?
Yes, but for CAV purposes only (DO48, s. 2017).
