FREQUENTLY ASKED QUESTIONS
The Assistant Schools Division Superintendent(ASDS) will approve application for leave of absence of teaching and non-teaching personnel with days of leave of absence not exceeding 60 days.
Schools Division Superintendent will approve application for leave of absence of teaching and non-teaching personnel with days of leave of absence exceeding 60 days, leave forms of School heads, PSDS/PSDI, section unit head, and monetization/terminal leave.
NOTE: Avoid erasures specially on the name of SIGNATORIES.
Official Authority to Travel shall be accomplished and submitted not later than FIVE(5) DAYS prior to the event or day of travel and a written JUSTIFICATION must be included to support the official travel. Approving Authorities shall consist signatory of recommending Approval of ASDS/head of the office for CID & SGOD , and Approval of SDS-DEPED ORDER No. 043 & 046 s. 2022.
Personal Authority to Travel shall be accomplished and submitted together with duly accomplished leave form( form 6). If the personal local travel is only for a period of three(3) days or less, a travel authorization is not required-DEPED ORDER No. 043 & 046 s. 2022.
NOTE: LOCATOR SLIP shall be use for activities/events/errands that would require leaving from or going out of the permanent station or workplace during office hours or within the day or travel not exceeding 1 day (24 hours), and travel will not take more than 50kms. The form should consist signatory of the requesting official/employee and APPROVED by the head of office or his/her duly representative-D.O. 043 & 046 s. 2022.

Authority to Travel Forms may be submitted walk-in at the the division office of Benguet-Records Section or submit online at records.sdobenguet@gmail.com.

Weekly School Work Week Plan shall be SUBMITTED prior to the implementation week and shall be submitted EVERY FRIDAY prior to the implementation week. The SWP shall consist signatories of the SCHOOL HEAD, REVIEWED BY PSDS, ASDS as RECOMMENDING APPROVAL, and SDS for APPROVAL-8.C. DM. No.381, S.2021.
IDLAR Shall be prepared at the END OF EVERY WORK WEEK indicating the accomplishment of agreed upon target and outputs as reflected in the School Workweek Plan. WEEKLY IDLAR of SCHOOL HEAD shall consist signatories of the SCHOOL HEAD, REVIEWED BY PSDS, ASDS as RECOMMENDING APPROVAL, and SDS for APPROVAL-8.D. DM. No.381, S.2021.
All incoming documents in the division office shall be submitted to the RECORDS section to be recorded and stamp with "RECEIVED".
