Provident Loan Requirements

1. Letter Request (State the purpose of the loan)

2. Loan Application Form (LAF) - 2 sets

3. Authorized to Deduct - 2 sets

4. Latest copy of Payslip - 1 original & 1 photocopy (must have NO undeducted accounts)

5. Photocopy of DepEd ID - 2 copies

Document/s to support the purpose of the loan :

1. Approved Appointment - 2 copies (Authenticated) for NEW applicants only

2. Updated Service Record - 2 copies (Authenticated) for NEW applicants only

Co-Maker

1. Position -must be level or higher than the borrower

2. Latest copy of Payslip - 2 photocopies only (must have NO undeducted accounts)
