SCHOOL FORM 1 - School Register
List of enrolled learners per class, including basic details such as name, LRN (Learner Reference Number), age, sex, etc.

SCHOOL FORM 2 - Daily Attendance Report of Learners
Tracks learners' daily attendance, used for monitoring and reporting attendance rate.
Note: Advisers must submit this form monthly.

SCHOOL FORM 3 - Books Issued and Returned
Records distribution and return of textbooks and other learning materials.

SCHOOL FORM 8 - Learner’s Basic Health and Nutrition Report
Monitors learners’ health and nutritional status. (Used particularly in schools implementing school-based feeding programs.)

SCHOOL FORM 9 - Learner Progress Report Card
Also known as the report card; provides learners and parents with updates on academic performance per grading period.
Note: To be submitted and updated by the class advisers at the end of every quarter, and must upload a scanned copy at the end of the school year.
SCHOOL FORM 10 - Learner Permanent Academic Record
Official transcript or permanent record of learners, includes historical academic and behavioral data.
