There are five (5) types of LIS transactions that require the action of the Division Planning Officer. To facilitate the approval of these division level requests, the LIS Coordinator/s or the School Registrar should ensure completeness and accuracy of the necessary documents consolidated from the class advisers and submit it to the division office by accomplishing the different google links depending on the type of request.
REQUEST FOR LRN APPROVAL
Every learner in the basic education system shall be issued only one permanent Learner Reference Number (LRN) that he/she shall use throughout the entire basic education program. The Search Learner Facility of the LIS shall aid in determining whether or not the learner already has an LRN. A learner's uniqueness must always be established before creating a new LRN.
This refers to all division level requests reflected in the "Data Correction Tab - LRN Approval as seen in the LIS account of the School Head and School System Admin (LIS Coordinator or the School Registrar)
For Grades 1 – 12 learners with no existing Learner Reference Number (LRN), the enrollment of the learner’s record in the LIS should be done by the class adviser or school registrar, the system will automatically reflect if there is an existing, LIS record for the learner’s basic profile provided. If there is none, the adviser will select the “create new record” button to proceed with the enlistment of the learner in the system. The learner’s name will appear in the class list although no LRN will be reflected instead it will be tagged as “FOR APPROVAL”.
For Kindergarten learner’s that will be enlisted in the LIS for the first time, the creation is automatically approved by the system. It is no longer necessary to request for Division approval. Kindergarten advisers should extremely be cautious in the encoding of the learner’s basic profile and must ensure completeness and accuracy vis-à-vis the leaner’s Birth Certificate. In case of wrong entry of the learner’s details (e.g. wrong spelling of name, wrong birthdate), the class adviser or school registrar should never create another record for the learner, a facility to correct the learner’s basic profile will be deployed in the system and is subject for School Head LIS account approval.
The request for LRN Approval will not appear in the LIS Data Correction Tab of the School Head/School System Admin/Division Office unless the learner is enrolled first in the LIS by the class adviser/school registrar.

CHANGE REQUESTS
This refers to all division level LIS requests reflected in the "Data Correction Tab - Change Requests" as seen in the LIS account of the School Head and School System Admin (LIS Coordinator or the School Registrar). This includes Enrollment with Gap, Enrollment of Ineligible, Correction of Grade Level and Correction of Basic Profile requests.
Enrollment with Gap
This occurs when the learner enlisted in the LIS for the current school year was not enrolled in the LIS in the previous year/s (Ex. Balik-Aral, learner previously enrolled in a non-accredited private school)
Enrollment of Ineligible
This occurs when the enlisted learner's end of school year (EOSY) status in the previous school year was not updated or was erroneously tagged.
Correction of Grade Level
This occurs when the learner was mistakenly enrolled in the wrong grade level
Correction of Basic Profile
This refers to requests related to the correction of learner's wrong entry of name and other basic information in the LIS. This may also pertain to the requests for LRN merging of learners with multiple LRNs.
The Change Requests will not appear in the LIS Data Correction Tab of the School Head/School System Admin/Division Office unless the learner is enrolled first in the LIS by the class adviser/school registrar.

REQUEST TO DISAPPROVE
This refers to requests by school to cancel the LRN approval requests or change requests reflected in their School Head/School System Admin's LIS Data Correction Tab.

EBEIS/LIS ACCOUNT CREATION
This refers to requests of school personnel and newly created Private Schools with no existing LIS/EBEIS accounts. It is a division requirement to facilitate the endorsement of creation of a school ID to the Regional Office.

This refers to requests related to the transfer of a school head's LIS/EBEIS account from one school to another and the resetting of School Head account's password.

The LIS Requests Forms for BOSY 2025-2026 update and enrollment is the official procedure to submit all LIS transactions that require the action of the Division Planning Officer. By entering your email address and other information, you give us permission to use your data to process your request.
Important Reminders:
1. Fill out all necessary information in the LIS request forms.
2. A DepEd email address is required. For non-DepEd requesters, a Gmail address is required to submit requests.
3. All updates on your requests will be sent to the email address provided.
4. Avoid submitting multiple requests with the same concern.
5. Resolution of issues is on a FIRST COME, FIRST SERVED basis.
