UNIFIED NLC DATA COLLECTION REPORT
AND WEEKLY DOCUMENTATION
Subject: Unified NLC Data Collection Report and Weekly Documentation Submission
On August 7, 2023, during the Monday Division Regular Meeting and CID Meeting with PSDSs, the following matters were deliberated and reached a consensus regarding the Unified DO-RO-CO NLC Data Collection Report and weekly documentation.
1. **Responsibilities and Data COLLECTION: **
· Part IA, Part IB, and Part II of the report are the responsibility of the School Heads.
· Part III of the report will be completed by the District Office through the PSDSs.
The specific data to be collected for each part are as follows:
· Sheet 1: Part IA: Number of elementary learners
· Sheet 2: Part IB: Number of secondary learners
· Sheet 3: Part II: Number of participating teachers in the NLC and other EOSY break activities.
· Sheet 4: Part III: Number of participating schools (for PSDSs)
2. **Reporting and DOCUMENTATION: **
· School Heads are required to upload their weekly school update reports on NLC activities to their respective district's folder.
· Within the district link, School Heads are instructed to create a subfolder for their school.
· Documents and photo documentation should be appropriately labeled and organized within the subfolder.
