2. Board Rating;

3. PRC License/ID;

4. Form 137, Transcript of Records and Diploma

D. Performance Rating in the last rating period with VERY SATISFACTORY rating (For Government Employees only)

E. Copies of Certificate of Training Programs attended

3. RENAME FILES IN THIS FORMAT: LastName_FirstName_Name-of-document

e.g. DELACRUZ_JUAN_PDS

4. All documents to be uploaded must be in PDF Format, maximum of 10MB per file.
