You will need to complete all of the linked forms in addition to the remaining items listed when registering at either Delsea Regional High School or Delsea Regional Middle School. 

When you have the following information ready, please contact our school registrar at ext. 275 to set up a time to come in and register. 

Note, copies of the forms below are available at the time of registration if needed.

1) New Student Survey (This survey must be completed in order for guidance to create your student's schedule).

2) Please return the following forms to Mrs. Dolan upon registration. New students for 2024-2025 should register after July 1st.

    Emergency Contact Form 

3) Record Request Forms - Regular Education Students / Child Study Team Students (IEP)

4) SEMI Form (This form must be printed, completed, and signed - please return it with your completed registration packet). 

5) Chromebook Acceptable Use Procedure and Form This Google Form must be completed. Please follow all instructions. If the student will be entering 7th or 10th grade, there will be a $30 warranty fee. This fee can be paid by cash, check, or online via the Paypal link that can be accessed after submitting the Form.

7) Transfer Card from Previous School  (if transferring from another school in NJ)

8) Health Records

9) Two Proofs of Residency- In Parent(s)/Guardian(s) Name.

Primary Proof (1) - Lease Agreement or Mortgage Statement // Deed// Settlement Paperwork or most recent Property Tax Statement

Secondary Proof (2) - Utility Bill issued within the last 30 days

10) Student Birth Certificate

11) Parent/Guardian Photo ID 

12) Custody Documentation  (if applicable)

13) Letter of Financial Responsibility (if applicable)

14) Language Survey

15) IEP/504 (if applicable)

16) Check for summer work here!
