All students with Asthma/Allergy MUST have a current Action Plan on file in SNAP
Medication Form must be filled out for ALL medications, including prescriptions, Epi-pens and inhalers. Also, if you would like the nurse to administer acetaminophen or ibuprofen for your student if needed.
Med Drop Off Night @DMS on Monday, 5/22 @6pm. All meds must be received by Tuesday, 5/23. You can drop them off any time during the school day.
Family Meeting @DMS on Wednesday, 5/24 @7pm

The DMS nurses will conduct grade-based health screenings , starting January, 29, 2024 ongoing through the remainder of the school year . For more information on MA state-mandated screenings you can click here. If for any reason you DO NOT want your student to participate in the state-mandated screenings, please fill out this form.
