This page of resources is specifically for students who are either currently enrolled in the Work-Based Learning Program or are considering applying for the program. The resources below will help you to better understand the Work-Based Learning program and the expectations we have for our students.

Before a student is allowed to leave DCHS to participate in an internship activity, the following items must be on file at DCHS and at the work site:

• Pages 9-11 of Personalized Learning plan with ALL required signatures

• Copy of proof of automobile insurance (if driving to internship site)

• Copy of valid TN Driver's License (or other proof of age)

• Copy of vehicle registration (if driving to internship site)

• Hazardous Employment Exemption form (if required)

• Satisfactory score on safety test

• Picture provided to instructor for badge (this will be your ticket through the Welcome Center.)
