Please complete this form for a Gifted and Talented Referral. 

The student ID number (lunch number) will be required. 

We recommend using the Google Chrome browser, if available. 

If you are a parent/guardian completing this form, you will be initiating the Gifted Identification process for your student to include all necessary evaluations (e.g., cognitive, achievement, observation & performance). 

*Within 30 school days, the Gifted Education Facilitator will reach out to confirm receipt of this referral form as well as provide updates and potential next steps.
