The following forms should be accomplished and submitted by the applicant (LCEs).

Step 2: Download, accomplish, scan, and upload the downloadable forms and documentary requirements*.

The applicant must also submit additional requirements listed below:

Updated Personal Data Sheet – CSC Form 212
Clearance/endorsement letter from concerned DILG Office or Office of the Governor.

Step 3: Once initial screening is done, successful applicants will be notified to undertake online assessments and interview to complete the application process.

Step 4: submit hard copies/original documents to dap office.
