Volunteer Clearance Information

Volunteers play a vital role in supporting the staff, students, and programs of the Dallastown Area School District. We are very thankful that you have expressed an interest in volunteering in our district.  In  order to volunteer for activities at DAIS, you must have approved clearances on file with DASD (if you have them completed for your place of employment or another volunteer position, you must still submit them to DASD to be added to the "Approved Volunteers" list)

The following documents must be on file with the District prior to the first day of volunteer service:

• Volunteer Information Sheet - Please complete and submit with appropriate documentation

• PA Child Abuse History (ACT 151) - Child Abuse clearance website - FREE

•  PA State Police Criminal Check (ACT 34) - 1-888-783-7972 - PATCH HelpDesk - Cost: FREE **Do not use Chrome or Apple phones or Macbooks.

• FBI Clearance Exemption Form - If you have lived in the Commonwealth of PA for 10 OR more consecutive years, you do NOT need to obtain a FBI clearance. Sign and date this form.

• FBI Clearance Website - Enter service code 1KG6ZJ - Cost: $24.20

In addition to completing the proper clearances, you will also need to review the information below regarding Mandated Reporting and sign the Acknowledgement form. Once all of the above has been completed, submit to the building secretary for approval.

• Mandated Reporter Acknowledgement - Sign and submit after reviewing mandated reporter information.

The Affidavit & Agreement listed below is to only be used for an immediate one-time volunteer date.  This form does not replace the above required documents.  You will NOT be added to the Approved Volunteers list until all five required documents are received and approved by the School Board. 

• Affidavit & Agreement - EXPIRES IN 90 DAYS.

Thank you for your interest in serving as a volunteer with the Dallastown Area School District. These volunteer clearance requirements further support the Dallastown Area School District commitment to providing a safe, secure learning environment for students.

Once completed, please turn in all parts to your child's community office.  They will be sent to HR and then approved at the next School Board Meeting.  Once approved, your name will appear on the "Approved Volunteers" list, where we will be able to confirm your status and you will be permitted to volunteer in the schools.
