What is needed to get a work permit?

1. Birth certificate (can be a copy or photo on phone)

2. Student getting the work permit must be present to sign 

3. Parent / legal guardian signature

The main office staff is presented the birth certificate and a form is completed that the parent / legal guardian needs to sign.  If the parent/legal guardian is not able to be present the student can take the form home to be signed. 

Once the form is signed by the parent / legal guardian the office staff will process the actual work permit.  The student getting the work permit must be present and sign this in front of the office staff.
