Dear Parents/Guardians:

On behalf of the District 25 Pre-K Center, we hope you and your family are in good health. Congratulations on being offered a seat to our PreK Center for the Fall of 2024.  

In order to proceed with Pre-Registration, we are asking that you take the following steps:

1. Fill out the Pre-Registration Form by clicking HERE. 

2. The following documentation will be submitted:

• • • A copy of your child's birth certificate, passport or record of baptism

• • • Two (2) verifiable proofs of address (in the name of the parent/guardian) which may include:

             - Lease agreement, deed or mortgage statement for the residence

             - Residential utility bill -- gas or electric (must be dated within the past 60 days)

             - Cable television bill (must be dated within the past 60 days)

             - NYS valid driver's license or learner's permit which has not expired

• Physical Form 

• • • Immunization Records

When Sending Your Email:

In the email subject please include your child's name along with "Pre-K Registration" (for example, John Smith – Pre-K Registration).
