Students who are 17 years old or younger MUST have an adult sign this form.
Students who are 18 or older must either have a guardian sign the form or they MUST sign it themselves.
All forms are due by Thursday, January 9.
Failure to complete this form by the due date will cause students to lose points in their PRAXIS assignment.
One point will be deducted for each school day after the due date that the form is not completed.
