ATTENTION ALL 5TH GRADE PARENTS:
A Certificate of Child Health Examination form is required for all 5th graders entering middle school (it may also be used for sports). Please be sure to have both sides of the form completed in its entirety by the physician. The health history portion must be filled out, dated, and signed by you in order for the form to be accepted for middle school entrance. In addition, an updated immunization record is part of the physical exam and must be included. Make a copy of the physical for your records. I will be accepting the completed forms in my office until May 26, 2023. After May 26th, please turn the forms in to the Principal’s office at the middle school. Do not mail them to the District Office.
A Sports Physical form is required on a yearly basis for any student participating in a sport. Please note that the sports physical is not acceptable for entrance into sixth grade. Please make a copy of the physical and turn it into the athletic office. A separate copy of the 6th grade physical must be given to the middle school nurse.
Dental Exam Form-A dental exam is required for all incoming 6th and 9th grade students.
Please begin to book your physicals and dental visits now as appointments during the summer months get filled quickly. Forms are available on the district website.
ATTENTION ALL 2ND GRADE PARENTS:
All dental forms should be turned in by May 15th.
ATTENTION ALL COTTON CREEK PARENTS:
As a reminder, students with a medical condition (diabetes, asthma, allergies, seizures) must submit an updated action plan and medication form prior to or at the start of school on August 16, 2023.
All medications must be picked up from the nurse’s office by Friday, May 26, 2023 by parents. Any medication not picked up by this date will be destroyed. If you have any questions, please call me at 847-526-4700 x 3405 or email me at wziolo@d118.org. Enjoy a healthy and safe summer!

Make sure you have up to date names and phone numbers listed for your children. The office would be happy to change or update these for you as needed. Your current contacts can be viewed in PowerSchool in the Forms section under the Enrollment tab. These contacts are essential if we need to reach you during an illness or emergency!

Please notify the office, as well as your teacher, if your student will not be in attendance. If there is a substitute, they do not have access to the teacher’s email.

Just a reminder, call the office at 847-526-4700 if:
1. You will be picking up your student early
2. They are a regular busser, but getting picked up or walking home that day.
