Registration Forms
All students that would like to participate in athletics at Williston Central School need the following completed by a parent/guardian before coming to any tryout or practice.
1) Online registration form completed Fall (Fall Sports and Basketball) and Spring for each student participating in Athletics. Spring Registration will open in January for Volleyball and Spring Sports. Sports registrations are closed for this school year. Unified Sports registration
2) All athletes are required to have had a current physical exam by a physician within the last TWO years, and have the Wellness Exam - Sports Participation Form (we also accept your Dr.'s version) filled out. It must be signed by their physician and submitted to the Athletic Director at the school each year in order for them to participate. Sports Well Form
3) Students will also need to have a current signed Sports Team Contract for each season. Sports Team Contract
Please note that registration does not mean athletes will make a team. There are tryouts and sometimes cuts. Please see the individual sports pages for more information on each sport.
