MEMBERSHIP

 Qualification:

Membership is open for employees of PSHS-CVisC (whether nature of employment is plantilla item, job order or contractual), outsourced employees of PSHS-CVisC and employees of the cooperative. 

Requirements:

1.       Approved Application form

2.       Certificate of completion of the prescribed Pre-Membership Education Seminar (PMES). (Answer this form)

3.       Subscribed and paid the required minimum share capital (Php 5,000.00), membership fee (Php150.00) and processing fee of (Php100.00)

4.       Anticipate using the services offered by the cooperative
