Registration

Interested Student-Athletes from both Wilcox Tech and Kaynor Tech, in order to participate, players need to have the following documents:

1. Current Sports Physical (Blue Form)

2. Connecticut Technical High School Interscholastic Permission Slip (White Form)

3. Sudden Cardiac Arrest Awareness Informed Consent Form (Green Form)

4. Exertional Heat Illness Program & Annual Review Form (Yellow Form)

5. Student and Parent Concussion Informed Consent Form (Purple Form)

6. Wilcox Athletic Handbook Sign Off (Orange Form)

7. Wilcox/Kaynor Football Player Expecations Agreement Sign Off (Pink Form)

All Paperwork is to brought in person to Chef Williams.
