Continuation of the Application Process

Please remember that your child’s continuation of the application process is predicated upon the following:

• •  Convening of a PPT/504 meeting (where applicable);

  • Final grades indicating successful completion of current grade;

  • Final attendance;

  • Discipline record that indicates no serious disciplinary infractions;

  • Immunization records;

  • Receipt and review of all records from the school in which your child is presently enrolled; (transcripts, discipline records, standardized test results, 504 Plans, special education records, school health records, psychological reports, social work assessments, record of extracurricular activities).

 ALL RECORDS MUST BE SUBMITTED TO THE SCHOOL COUNSELING OFFICE BY JULY 5, 2023.

Your child must also have a Physical Health Exam (not to be completed until AFTER July 1, 2023, but BEFORE June 30, 2024).
