Percentage of the Program Educational Objectives attained/achieved by the graduates;
Percentage of graduates who were employed and percentage of graduates who became successful entrepreneurs;
Feedback from employers regarding the performance of the graduates in their respective workplace;
Number of graduates who are given awards and recognition for outstanding performance/contributions in the field/ workplace; and
Number of prominent graduates who have outstanding contribution in the field of information technology.
Include in the documentation the process of evaluating the appropriate beneficiaries for the extension project This will strengthen the extension team's claim that positive outcomes are anticipated from the extension initiatives.
Evaluate and revise the specific goals of the extension project to include the actual implementation of training.
Provide a schedule detailing how the conduct of training will be implemented for targeted clients.

b. Endorsement letter to the IM Committee Documentary proof of the IM's review, editing. and feedback from the IM Committee
c. The final copy of the IM which has been approved and endorsed by the IM Committee, the Dean of the College, and the University IM Committee.
d. Proof of submission and copyright approval for the approved instructional material.
