Are you a Google Guru of Forms?

1. Create a Form to survey students on how the course is going (formative mid semester evaluation)
2. Set Options:
   1. Collecting Responses- require login?, one response/person, randomize questions
   2. Theme
3. Publication: Launch your form for responses
4. Set up Spreadsheet under Responses to collect in a Google Sheet
5. Share the link from within the form via email to atol@css.edu
