Through this clinical experience, I learned how to obtain basic assessment data in order to guide, prioritize, and implement my nursing interventions. I performed fundamental skills such as medication administration, assisting with activities of daily living, and proper documentation.
Simulation allowed me to apply the theoretical knowledge I had learned in a safe and controlled environment that allowed me to learn from my mistakes. I used my technical and critical thinking skills learned in didactic and lab to obtain assessment data in order to guide my care, perform basic procedures, and communicate effectively with my patients, peers, and interdisciplinary team.

Through this simulation and virtual clinical experience, I was able to implement therapeutic communication in order to provide optimal care for my patients that are living with an altered pattern of behavior, thought, or mood. I learned about the multiple presentations of behavioral health issues and the importance of helping clients to feel safe and heard. In the controlled learning environment, I learned how to deescalate situations in order to foster a safe environment for the patients and staff, beginning with the least restrictive interventions to promote ethical care.
Population Health - Cultivate Initiatives (90 Clinical Hours)
In this clinical experience, I had the opportunity to work with underserved and houseless individuals living in the community. I assessed vitals signs, performed wound care, and provided education and resources for continuity of care. I also exercised my therapeutic communication skills foster a non-judgmental environment. This experience opened my eyes to the health disparities and social determinants of health of those living in the community.
Nursing Care of the Adult I - Gresham PAVU Skilled Nursing Facility (90 Cumulative Contact Hours of Clinical and Simulation)
Through this clinical experience, I was able to apply my knowledge that I had gained in my first term of nursing school by conducting comprehensive head-to-toe assessments and using my data to guide my plan of care through the use of evidence-based knowledge. In addition to this, I performed skills such as medication administration, wound care, and tracheostomy care. Being on ventilator unit, I learned how to assess nonverbal cues and assessment data to tend to the needs of the patients and ensure comfort.
Through the simulation environment, I learned how to recognize changes in client condition and provide quick, appropriate interventions to prevent deterioration. I also learned how to use assessment data to guide care and identify the potential risks the client faced as a result of their condition and how to prevent further complications.
Nursing Care of the Adult II - Providence Portland Medical Center (90 Cumulative Contact Hours of Clinical and Simulation)
Through this clinical experience, I was able to refine my assessment skills by performing comprehensive and focused head-to-toe assessments and using the data to guide my prioritization, implementation, and evaluation of patient care. I monitored and interpreted cardiac telemetry strips, administered medications, performed client education and discharge teaching, and practiced thorough and accurate documentation. Working in a hospital allowed me to collaborate more with interdisciplinary teams to provide a holistic approach to client care.
Through the simulation experience, I learned how to provide thorough client education and evaluate my teaching to ensure the client understood the importance of our interventions. Similarly to Nursing Care of the Adult II, I learned how to recognize signs, symptoms, and assessment data to provide timely interventions to avoid deterioration and improve the client's condition. I also learned the importance of following hospital protocol and working as a team to promote optimal patient outcomes.

Through this simulation and virtual clinical experience, I understood the importance of taking a family-centered approach to pediatric and obstetrics nursing. When it comes to pediatrics, I learned that parents can be valuable sources when it comes to obtaining subjective data for pediatric patients. I also learned the importance of using age appropriate communication and interventions, and properly calculating medication dosgaes based on weight. In obstetrics, I learned that there are two patients to care for, and being sure to provide care that aligns with the patient's wishes while also prioritizing the best outcomes for both the parent and child. Through these experiences, I was able to assess for potential warning signs and implement appropriate interventions to prevent and treat complications.
Age, Chronicity, and Palliative Care - Gracelen Terrace Skilled Nursing Facility (45 Cumulative Contact Hours of Clinical and Simulation)
Through this clinical experience, I applied my assessment skills to provide care that met the physical, emotional, and spiritual needs of the patients. I also was able to provide therapeutic communication to patients and their families in an effort to provide emotional and psychosocial support in this final stage of life. I was provided with the opportunity to implement comfort care measures, whereas previous clinicals had focused on treatment. In addition to this, I also administered medications, assisted with activities of daily living, performed wound care, and practiced thorough and accurate documentation.
Through the simulation experience I was able to provide therapeutic communication and support to the patient and their family in the final stage of the patient's life. I answered their questions and ensured that the experience aligned with the wishes of the patient while also considering the emotional needs of the family member. I learned of the appropriate and necessary actions that take place by the nurse after a patient has died.
Transition to Practice - Kaiser Skyline Medical Center Pediatrics (225 Cumulative Contact Hours of Clinical and Online Work)
Through this clinical experience I was provided the opportunity to use age-appropriate communication to build rapport, provide comfort, and obtain comprehensive assessment data of pediatric patients. I used my data to monitor appropriate growth and development milestones and coordinate with the provider and other interdisciplinary team members to implement appropriate interventions. As part of the care for pediatric patients, I conducted newborn assessments which included obtaining transcutaneous bilirubin measurements and assessing the lactation and feeding schedules, and providing client education on feeding and lactation to the parents. In addition to these tasks, I also performed lab specimen collection for further diagnostics, scheduled and coordinated patient care, and documented my findings, ensuring thoroughness and accuracy.
