Functional Behavior Plan (FBA) Process
1. Obtain INFORMED CONSENT to start the process of a Functional Based Assessment(FBA).
2. Record Review (Gather the following information):
a. SOCIAL HISTORY
b. DISCIPLINE RECORD
c. PSYCHOLOGICAL EVALUATION REPORT
d. OUTSIDE MENTAL HEALTH EVALUATION REPORT/DIAGNOSES
3. Complete the Behavior Assessment System for Children (BASC-3) rating scale that will be sent via email from the School Psychology Specialist
4. Define the reason for the FBA and complete the PROBLEM BEHAVIOR QUESTIONNAIRE and TEACHER INTERVIEW
5. Teacher will begin collecting baseline data (observe problem behavior for 5 days) using the FREQUENCY/RATE form.
6. Teacher or other school personnel will begin doing observations using the ABC CHECKLIST form. At least 3 observations in different settings are required for each FBA.
7. Submit ALL of this information to the School Psychology Specialist. The SPS will analyze this data and determine the function of the problem behavior and suggest 1-2 possible interventions based on the BASC-3 ratings.
8. Teacher will work with student to complete a REINFORCEMENT SURVEY, and then develop a Behavior Intervention Plan.
