1st-8th Grades

Registration for 1st through 8th grades for the 2025/2026 school year will begin after August 5, 2025.  Use the CPS School Locator to ensure Mark Twain is your neighborhood school.

Please fill out THIS FORM if you live in the Mark Twain attendance area and plan on enrolling your child at Mark Twain School.  Fill out a separate form for each child you will enroll.

A member of the office staff will contact you to schedule an appointment for registration.  Any questions regarding registration at Mark Twain School should be emailed to twainregistration@cps.edu

Requirements:

1. A parent or legal guardian must enroll the student.

2. The parent or legal guardian must have 2 pieces of legal identification with current address

   • driver's license or state ID

   • current utility bill

   • insurance statement

   • bank statement

   • paycheck stub

3. Transfer documents from previous school (and ISBE form if outside of CPS)

4. Health Exams and Immunizations: Minimum Health Requirements 

   • Important Note: A current physical is required at the time of registration.

5. If new to Chicago Public Schools: 

   • the child's birth certificate is required 

   • a transfer from the student's previous school is required

   • a current physical exam

   • a current vision exam

   • a current dental exam
