For the upcoming school year, Kindergartners must be age 5, born before September 1, 2019.

Please fill out THIS FORM if you live in the Burley attendance area and your child will be 5 years old before September 1st, 2024 to pre-register your student.

Pre-registration appointments for Kindergarten will begin Monday, April 15, 2023. Use the CPS School Locator to ensure Burley is your neighborhood school.

A member of the office staff will contact you to schedule an appointment for pre-registration. Any questions regarding registration at Burley School should be emailed to burleyregistration@cps.edu

Requirements: 

A parent or legal guardian must enroll the student.  If the student is coming from a CPS preschool or CPS elementary, please request a transfer from that school.

Proof of current address includes, but is not limited to, any two of the documents listed below:

• Illinois driver’s license or State of Illinois identification card (preferred by the school)

• Current utility bills

• Deed

• Employee identification card

• MediPlan/Medicaid card

• Voter registration card

• Court documents

• Illinois Department of Public Aid card

• Stamped United States Post Office change of address form

• Illinois state aid check/social security check

• Other identification card issued by a federal or state agency or a foreign government consulate, such as a Matricula Consular

Proof of age includes, but is not limited to, one of the documents 

• Child’s original birth certificate

• Child’s baptismal record (not accepted for preschool enrollment)

• U.S. or Foreign Passport

• Court documents (accepted for preschool enrollment — foster children only)

• Medical records (not accepted for preschool enrollment)

*State-issued medical card also accepted for preschool enrollment only.

Health Exams and Immunizations: Minimum Health Requirements 

1. 1. Health Exam Form (required): Physical Examination requirements due upon enrollment.  Physical Examination must be completed within 1 year prior to entry and are required for all students entering Kindergarten.

   2. Dental Exam (required): Dental exam within 1 year due upon enrollment for all students entering kindergarten

   3. Vision Exam (required): Vision exam within 1 year due upon enrollment
