A parent or legal guardian must enroll the student.

driver's license or state ID
current utility bill
insurance statement
bank statement
medical card
paycheck stub
3. Transfer documents from previous school (and ISBE form if outside of CPS)
4. Health Exams and Immunizations: Minimum Health Requirements
Important Note: A current physical is required at the time of registration.
