Complete your paper work and turn it into the coach the first day of practice. All practice times and locations are posted on the Pride Calendar. Check back often as practices are updated!!
Remember to make copies of all forms and keep one set for your personal records
Sports Physical is good for one year and is good for all sports.
It must be on this PSAL form and stamped by the doctor.
Each sport will need a new parent consent form.
This is new for the 2021 School Year.
Please make sure you fill it out in addition to all two other standard forms
