Due to a high volume of testing, The Charter Oak Unified School District will only be offering free COVID-19 testing for students and employees.
If you are not a student or employee, free testing is also offered at Wingate/Kahler Russell Park at
735 N Glendora Ave, Covina, CA 91724. To make an appointment Click Here.
COVID Testing Information
Below you will find all the information, forms, and links to participate in COVID testing at COUSD school sites. Click on the BLUE button to open the link.
In order to test, you will need to complete the following:
COVID Registration (use the school code listed)
Patient Test Request Form (must be filled out every time you test)
Parent Consent Form (if the participant is under 18 years old)
Proof of Insurance - you are required to present insurance information at the testing site OR submit the Uninsured Patient Form (link provided below) and bring a copy with you to the testing site.

Reference this sample form for questions in completing the registration.
The patient test request needs to be printed, filled out, and brought EACH time you test.
Parent Consent Form must be filled out for all students/athletes under 18. This form provides consent for the duration of the school year, until June 2022. Print and sign and bring to the testing site.
This form only needs to be submitted one time. It will be kept on file until June 2022.

This form is used only for those testing who do not have insurance, and/or did not register their health plan on the PMH Website.
This form is only filled out once and given to the PMH Lab nursing staff upon the first test.
COVID Testing Lab Results
PMH Labs will NOT provide test results over the phone. Results can be accessed through this link. You will also receive an email with a link within 24-72 hours of testing.
PMH Labs states that their typical turnaround is 24 hours however, they have up to 72 hours to provide the results in case of delays.
