Complete and submit the following onboarding forms no later than 8 AM CT, Friday, April 25, 2025. These forms are needed to establish your UH identity, set you up for stipend payments, and travel reimbursements.

“Unique Identifier” – select any of the options (social security number, driver’s license information, etc.) and provide the requested information. If you have concerns about providing your Unique Identifier in written form, you may provide the information verbally. Please email Matthew Pariyothorn at mmpariyo@central.uh.edu with your concern, so that we can schedule time for a brief phone or virtual call.

Download the Participant Cost Form and complete the following sections:
Payee Name (Last, First MI)
Payee ID # (for UH students) – input your my UH ID/PSID; Leave blank if you are not a UH system student.
Phone #
Address where a check can be Mailed (your REU stipend will be direct deposited; however, this mailing address is still required as a backup)
Payee Classification
Non-UH students should select “non-UH employee”.
UH students (all campuses – Main, Downtown, Clear Lake, Victoria) should select “UH student”. UH students cannot be employed while in the REU program.
If you are a UH student (all campuses)
Select your UH student classification as “Undergraduate”
Select your citizenship type.
Indicate whether you are currently receiving financial aid for the 2024-2025 school year, by selecting either N or Y in the section labeled “I am receiving other financial aid”. If selecting Y, please review the additional instructions in this section.

Download the Foreign National Addendum
Select “N/A – I am a U.S. Citizen or Permanent Resident (green card).”

Input your Name (Last, First, Middle).
If you selected “N/A – I am a U.S. Citizen or Permanent Resident”, please ignore the sections requesting “Country of Tax Residency, Country Issuing Passport, Permanent Street Address, City, State or Providence, Zip or Postal Code, and Country.”
In the Check box section, select both “Student Travel Grant or Travel Reimbursement” and “Other”. To satisfy the “Other” selection, specify by writing “Undergrad research training stipends”.

This is not a paper-based form. In the following days, you will receive an email invitation from the UH Vendor Registration portal called “PaymentWorks”.

It is important that you complete the vendor registration process as soon as you receive the email invitation.

Include a screenshot of your preferred flights (roundtrip) and the airline's cost of checked baggage.
Only domestic economy flights are permitted (e.g. Economy/Coach/Wanna Get Away)
Process:
With the requested information, Amber will create your travel profile in the Concur Travel system.
Once your Concur travel profile has been created, she will submit a travel request on your behalf.
Once the travel request has been approved, Amber will inform you that you can make travel arrangements.
Form Submission Instructions:
For security reasons, please encrypt the ePOI form. If you prefer, you may encrypt all 3 forms (forms 1, 2, 3 above).
