Parents/Guardians can use the form below to refer a student for non-emergency counseling services. Responses will be checked Monday through Friday from 8:00 am to 3:00 pm. If you need emergency mental health support, tell a trusted adult near you and call 911, your local hospital emergency department, or the National Suicide Prevention Lifeline at: 1-800-273-TALK (8255), or text "HOME" to 741741
