Support CHS PTSA by becoming a member!
Every year, our PTSA has the privilege of choosing both an outstanding educator and an outstanding volunteer that represents our school. The first step in this process is the nomination of individuals. Anyone may send in a nomination. Below are the criteria and guidelines for nominations. If you would like to nominate someone, please complete the appropriate forms (linked below). You may attach an additional sheet of paper to the nomination form.

Honors volunteers who exemplify the PTA mission in the service of students, families, and the community at your school. Nominees must be involved in and support your PTA/PTSA, have made a positive impact on the lives of children, and be dedicated to the mission of PTA (To make every child’s potential a reality by engaging and empowering families and communities to advocate for all children).

Honors teachers, administrators, and certified staff who exemplify the PTA mission in the service of students, families, and the community at your school. Nominees must be involved in and support your PTA/PTSA, have made a positive impact on the lives of children, and be dedicated to the mission of PTA (To make every child’s potential a reality by engaging and empowering families and communities to advocate for all children).
