Schools help all students prepare for their future. If a student has a disability and receives special education services, there is a process called transition planning included in the Individualized Education Program (IEP) by the time they reach age 14. This process can help decide what a student and family want for the future and figure out the steps to take to meet those goals. Transition planning can help the important people in a student's life, such as family and teachers, understand more about what the student wants for their life when they become an adult.
