Each student must submit the required forms through STEM Wizard. All students are required to submit the following forms:
Form 1 (Checklist for Adult Sponsor)
Form 1A (Student Checklist)
Form 1B (Approval Form)
Depending on the project, additional forms may be necessary. Additional forms include:
Form 1C (Regulated Research Institutional/Industrial Setting)
Form 2 (Qualified Scientist)
Form 3 (Risk Assessment)
Form 4 (Human Participants)
Form 5A (Vertebrate Animal)
Form 5B (Vertebrate Animal - Regulated Research Institution)
Form 6A (Potentially Hazardous Biological Agents Risk Assessment)
Form 6B (Human and Vertebrate Animal Tissue)
Form 7 (Continuation/Research Progression Projects)

Texas Science and Engineering Fair has streamlined the safety form process for junior high students. The single form will cover all signatures for a student's project.
Students are not required to have every signature on the form. Signatures required depend on the project and safety requirements for each project.
Human Participant Projects:
School Administrator
Counselor (for non-physical projects)
School Nurse (for physical activity or ingesting projects)
Vertebrate Animal Projects:
Veterinarian MUST sign off on the project. These signatures are getting hard to acquire, so please take note of that from the beginning of the process.
Similar to the high school forms, all digital signatures must be certified and verified.
