After you have created the Google Form that you are satisfied with: Use the goo.gl Chrome Extension to create your Shortened Link and QR code for the Parent Letter

Make certain when capturing the link for the Parent/Guardian Letter you are on the /viewform link and NOT the /edit version or the Parent/Guardian will NOT be able to access the form.
