If you are having trouble filling out these forms, please consult a parent/guardian.

If all information is submitted correctly by the deadline, you will receive a confirmation email with next steps and further information on the following Tuesday! 🎉
⚠️ Please Note: A green check mark does NOT mean that you have uploaded a valid document. You may be contacted about if your document is invalid.
Make sure that all documents are VALID. We cannot accept any documents that are expired or not visible.
If a document expires before or during the summer, please be prepared to submit a replacement document. Failure to do so will result in suspension from the program or possible removal.
Please pay attention to which documents require a signature. If you do not sign documents that require it, we cannot accept that document until it is signed.
