Form 1: Emergency Medical Form
The Pre-Participation Physical Evaluation Form has been updated to include the physician's physical, Head's Up Concussion Form, and the Lindsey's Law (Sudden Cardiac Arrest Form). Students and parents/caregivers MUST watch the Sudden Cardiac Arrest Video (posted below) for eligibility.

The Columbiana Participant Code of Conduct is the Board Adopted Athletic and Extra-Curricular Activity Handbook. Both student and parent/caregiver must read the Code of Conduct and complete the agreement prior to participation.

Request forms are for any anticipated absence or tardy. This form may be adjusted to an online Google Form.
Please use this page as a guideline for the established apparel expectation.
All student performers are required to wear black dress pants accompanies with black dress shirt. The dress shirt may be a black button down dress shirt, black blouse, or equivalent.
Male performers must include a black dress belt, black dress shoes, and black dress socks. Males may accompany their black dress shirt with a solid black tie (optional).
Female performers are permitted to wear a closed or open-toed black dress shoes, black dress flats, black heels, or black dress anklet boots (calf-high or knee high boots are not permitted).
All concert apparel MUST meet the school dress-code standard.
Apparel NOT PERMITTED:
Jeans, Yoga Pants, Leggings, Sweats, Shorts, Capris, Clothing with words/graphics/printing, Skirts, Dresses.
Tennis Shoes, Ugg Boots, Slippers, Sandals, etc.
White or no-show socks.
